Recovery Care

with Face Down Chairs is a key part of your treatment and safety. Be sure to make and go to all appointments, and call your Doctor or Nurse line if you are having problems.

01. Will my insurance pay for this?

You will pay out of pocket at the time of delivery, but FDC will supply you with the necessary forms to submit to the insurance company for potential reimbursement.

02. How do I submit forms for reimbursement?
1.  FDC will fax the Certificate of Medical Form (CMN) to your Doctor.  
2.  Your Doctor’s office will fax it back to FDC.
3.  FDC will return these completed forms to you via email or snail mail. (Fill in your personal info)
4.  Consult with your insurance provider on where to mail the forms for a reimbursement request.
5.  Send the forms provided by FDC.
6.  Be sure to follow up!

03. When will I receive the forms to submit for reimbursement?

Please allow 1-2 weeks for your forms to arrive after the equipment has been retrieved.

04. How much will I have to pay?

The payment will vary depending on which package you select.

05. How will I eat?

You cannot properly consume food in a face down position. You will be allowed a short break every hour to eat.

06. Can I sleep in the Vitrectomy Chair?

You may take a 15-20 minute nap, but it is not advised rest longer than that period.

07. I did not like the equipment or only used a portion of it, may I get a refund?

No refunds will be given once the delivery technician leaves your home.

08. My Doctor finished surgery, then decided that I did not have to position, may I get a refund?

Our 50% refund policy applies if you contact us the day of surgery to cancel. We do not offer full refunds.

09. When will the equipment be picked up?

Call Face Down Chairs once your Doctor has cleared you from positioning.

10. Do I need to be there for the delivery?

Option 1: We can show a family member how to operate the equipment.

Option 2: We can leave it on a front or back porch (if hidden out of sight).  The patient can then watch the tutorial video for set up.

11. Can I watch T.V. while sitting in my Vitrectomy chair?

Yes, if you rent the mirror, you will be able to see the t.v. in the correct orientation.
