HOW IT WORKS
01.
Face Down Chairs will fax the Certificate of Medical Necessity (CMN) form to your Doctor.
02.
Your Doctor’s office will complete the Certificate of Medical Necessity Form (CMN) and fax it back to Face Down Chairs.
03.
Face Down Chairs will return these completed forms to you via email or by United States Postal Service.
04.
Consult with your insurance provider on where to mail the forms for reimbursement request.
05.
Send the forms provided by Face Down Chairs.
06.

PRIOR TO SURGERY
PRIOR TO SURGERY:
Call your insurance carrier customer service pre-authorization line. When asked for the code, please give E1399. If necessary, they may call Face Down Chairs on a 3 way call at (470) 242-4733. Please let us know ahead of time, so that we can prepare.
