Will my insurance pay for this?
You will pay out of pocket at the time of delivery, but FDC will supply you with the necessary forms to submit to the insurance company for potential reimbursement.
How do I submit the forms for reimbursement?

Your provider will give you the address to mail it to. **You may need to attach a claim form, in addition to what FDC supplies. When will I receive the forms to submit for reimbursement?
Please allow 1-2 weeks for your forms to arrive after the equipment has been retrieved.
How much will I have to pay?
The payment will vary depending on which package you select.
How will I eat?
You cannot properly consume food in a face down position. You will be allowed a short break every hour to eat.
May I sleep in the Vitrectomy Chair?
You may take a 15-20 minute nap, but it is not advised rest longer than that period.
I did not like the equipment or only used a portion of it, may I get a refund?
No refunds will be given once the delivery technician leaves your home.
My Doctor finished surgery, then decided that I did not have to position, may I get a refund?
Yes, all but $50 will be refunded, provided that you have written proof from your Physician and that you contact FDC within 24 hours of your surgery date.
When will the equipment be picked up?
We will schedule that appointment at the time of delivery.
Do I need to be there for the delivery?
No, we can show a family member how to operate the equipment.
May I clean the equipment?
The equipment is sanitized before the initial delivery, but feel free to use a sanitizing wipe before use.
May I take the equipment to another location in my home?
Yes, the equipment is compact and able to fit in most spaces comfortably. However, the Vitrectomy Chair is 24 lbs. and should not be moved by the Patient.
I am unable to bend my knees, can the knee rests be removed?
Yes, they are removable.
Can I watch t.v. while positioning?
Yes, if you rent the mirror, you will be able to see the t.v. in the correct orientation.
Can I use my laptop with the Vitrectomy Chair?
A laptop will not fit on the arm rest, but you can place it on a platform in front of the chair for use.
Survival Tips
- Prepare your meals in advance.
- Use a heating pad or an equivalent of Icy Hot on your shoulder and neck region.
- Download Audible (books online).
- Find a series to watch on Netflix.
- Have a table or tray next to you to have your necessities near by. I.E. cell phone, drink with a straw.
- Place a stool or table under the sleeping head support to use your phone or tablet.
- Use pillows underneath your abdomen and feet for comfort.
- Attach a tennis ball or empty water bottle to the back of your pajama shirt to prevent rolling onto your back.
- Fresh facial tissues for excess drainage.
- Buy paper tape for your eye shield to be used at night.
- Wear button down or loose shirts.
- Men should shave prior to surgery.
- Women should wear their hair up.
- Use a detachable shower head to avoid wetting your eye after surgery.
- Stretch on your break.
